Ive messed around with it, the main difference I find is that with an extension such as ublock origin, the page displays more normall as if the ads were never there, whereas using the adguard DNS, there is a bunch of white spaces to scroll through where the ads are supposed to be.
This is more of an issue with mobile devices i find, half the page is white.
Also sponsored links dont open, not really a surprise here, but a minor inconvenience at times when you have to scroll further down to find the direct link.

Was watching this here last night, quite an eye opener on whats going on currently.
